Proper way to set a formal table

1. Napkin 6. Dinner Plate 11. Cocktail Fork
2. Salad Fork 7. Dinner Knife 12. Water Glass
3. Dinner Fork 8. Teaspoon 13. Red Wine Glass
4. Dessert Fork 9. Teaspoon 14. White Wine Glass
5. Bread & Butter Plate 10. Soup Spoon 15. Coffee Cup & Saucer*

Note: this is a very formal setting. You shouldn't put out utensils that won't be used. Also, if you're planning to serve coffee and dessert afterward, bring out the teaspoons, dessert forks and coffee cups then.
